The Provider Relief Fund (PRF) Request to Report Late Due to Extenuating Circumstances process is for providers who were required to report in an applicable reporting period, but extenuating circumstances prevented them from doing so by the required deadline. PRF Period 2 (when payment was received) covered July 1, 2020, through December 31, 2020, with an associated reporting period (RP2) of January 1, 2022, to March 31, 2022. The deadline to request to report late for RP2 is Friday, May 13, 2022, at 11:59 p.m. ET.
All providers who are considered non-compliant (i.e., did not report during the required reporting period) will be notified by Health Resource and Services Administration (HRSA) after the conclusion of the reporting period and be given details on how to submit a request. Providers who did not submit a RP2 report and are not contacted by HRSA should visit the HRSA website or call (866) 569-3522. Providers will submit a Request to Report Late Due to Extenuating Circumstances form indicating the extenuating circumstance and attesting to a clear and concise explanation. Supporting documentation will not be required.
Providers whose request is approved will have the opportunity to complete their report in the PRF Reporting Portal. Providers whose request is denied will remain non-compliant with the Terms and Conditions and will be required to return all funds to HRSA that were not reported on in the applicable reporting period. HRSA will not require non-compliant providers to return funds until after the Request to Report Late Due to Extenuating Circumstances processes for RP1 and RP2 have closed.
Submission of a request does not guarantee the request will be approved or that a provider will be allowed to enter the PRF Reporting Portal to complete and submit a report. Approval or denial of requests are subject to determination by HRSA. Providers whose request is approved will receive a notification to proceed with completing their report and will have 10 days from the date the notification is received to submit a report in the PRF Reporting Portal.
Approval of Late Reporting Requests for Reporting Period 1

Providers who submitted a request will be notified if their request is approved starting the week of May 9, 2022, and they must submit their completed report within 10 days of receiving the approval notice.
