The American Society of Clinical Oncology is accredited by the Accreditation Council for Continuing Medical Education to provide continuing medical education for physicians.
The American Society of Clinical Oncology designates this educational activity for a maximum of 22.75 AMA PRA Category 1 CreditsTM. Physicians should only claim credit commensurate with the extent of their participation in the activity.

Program Evaluation & CME Certificate Request
Physicians must submit an evaluation by February 24, 2010 in order to receive CME credit for attending this Symposium.
Nonphysicians (including nurses and pharmacists) must submit an evaluation by February 24, 2010 in order to receive a Certificate of Participation, which may enable nonphysicians to apply their Symposium participation toward re-licensure.
Evaluations may be accessed through www.asco.org/cme.

CME Certificate Retrieval
Certificates will be available March 24, 2010 and will NOT be mailed to any attendee. E-mail notifications will be sent to those attendees who have completed evaluation forms, with instructions on retrieving certificates online:
ASCO Members: Use ASCO.org login OR Badge Number and Last Name to access certificates on the CME Bank. Certificate notifications will be sent to the e-mail address in your ASCO member record. You may update your e-mail address at www.asco.org/myasco at any time.
Nonmembers: Use Badge Number and Last Name to access certificates through the Single Certificate Retrieval. Certificate notifications will be sent to the e-mail address used during the registration process. If you need to update your e-mail address, please send changes to giregistration@jspargo.com .
